As an eligible employee, you receive Basic Life and AD&D insurance equal to 2 times your annual base salary up to a maximum of $750,000. Basic Life and AD&D are provided by the company at NO COST to you.
You may choose anyone to be the beneficiary of your Life and AD&D policies. Review your beneficiary designation periodically to make sure it reflects your current wishes. You can change your beneficiary designation at any time by logging into Workday, clicking on the Benefit icon from your home page, and selecting the “Change Beneficiary” option.
In addition to Basic Life and AD&D, you may purchase Voluntary Life and AD&D coverage at discounted rates for yourself, your spouse, and your child(ren).
Coverage exceeding the guaranteed issue amount or purchased outside of your initial enrollment period is subject to Evidence of Insurability (EOI). EOI is not required for AD&D coverage.
STD coverage provides you with a portion of income replacement if you are unable to work due to a non-occupational illness or injury. You are automatically enrolled in the STD plan at NO COST to you.
Company-Paid STD Coverage: 66.67% of your base weekly pay, up to $2,500 per week, for up to 90 days, after a 7-day waiting period.
STD benefits may be offset by the benefits you receive from state-mandated disability plans.
LTD coverage pays you a portion of your earnings if you cannot work for an extended time due to a disabling illness or injury. You are automatically enrolled in the LTD plan at NO COST to you.
Company-Paid LTD Coverage: 60% of your monthly earnings, up to $15,000 per month, after 90-days of continuous Short-Term Disability, up to Social Security Normal Retirement Age.
Benefits are reduced by other sources of disability income you may qualify for, such as Social Security and Workers’ Compensation.
You can purchase Voluntary Long-Term Disability Buy-Up coverage in addition to the company-paid LTD plan. LTD Buy-Up premiums are payroll deducted.
Voluntary LTD Buy-Up Coverage: Additional 6.67% (for a total of 66.67% of your base pay when combined with the Basic LTD plan coverage), up to $15,000 per month collectively, after a 90-day waiting period, up to Social Security Normal Retirement Age.
Accidental Injury, offered on a voluntary basis through Cigna, provides coverage for specific injuries and treatments resulting from a covered accident. The amount of the benefit paid depends on the type of injury and care received. You can also receive an annual wellness benefit of $75 for completing a routine mammogram, certain blood tests, and a preventive or gynecological exam.
Since benefits are paid directly to you, you choose how to use them, such as paying medical bills, subsidizing lost income, or covering everyday expenses.
Critical Illness coverage, offered on a voluntary basis through Cigna, pays you a $20,000 lump sum benefit if you are diagnosed with a covered illness or condition. All benefits are paid directly to you and you may use the funds as you see fit.

Benefits from the Chubb Travel Accident Program are paid in addition to any amounts paid from your Basic and Supplemental Life and AD&D plan(s), as applicable. Your spouse, domestic partner, and eligible dependent children are also covered when they accompany you on authorized business travel.
